Περίληψη
O στόχος της παρούσας εργασίας είναι η τεκμηρίωση της αναγκαιότητας και της αποτελεσματικότητας της πρώιμης παρέμβασης στην περίπτωση της ΔΕΠ-Υ, μέσα από την παρουσίαση των κυριότερων προγραμμάτων πρώιμης παρέμβασης τα οποία έχουν εφαρμοστεί διεθνώς, καθώς και ενός προγράμματος το οποίο εφαρμόζεται στη χώρα μας στο πλαίσιο της πολυεπίπεδης προσέγγισης στην αναπτυξιακή ψυχοπαθολογία. Τα προγράμματα αυτά αντλούν θεωρητικά από το βιοψυχοκοινωνικό μοντέλο, σύμφωνα με το οποίο η ΔΕΠ-Υ δεν είναι απλώς το αποτέλεσμα δομικών ή λειτουργικών νευροβιολογικών ελλειμμάτων, αλλά η κατάληξη μιας αναπτυξιακής διαδρομής η οποία χαρακτηρίζεται από την αλληλεπίδραση βιολογικών και περιβαλλοντικών παραγόντων που επηρεάζουν τη δομή και λειτουργία του εγκεφάλου νωρίς κατά τη διαδικασία της ανάπτυξης. Η πρώιμη παρέμβαση στοχεύει στις διαδικασίες που λαμβάνουν χώρα πολύ νωρίς στην αναπτυξιακή πορεία και έχουν αιτιολογική σχέση με τη ΔΕΠ-Υ, με στόχο να τροποποιήσει την υποβόσκουσα νευροφυσιολογία της και να παράγει σταθερή και γενικευμένη αλλαγή, «διορθώνοντας» τις αναπτυξιακές διαδρομές που οδηγούν στην εκδήλωσή της. Η αποτελεσματικότητα της πρώιμης παρέμβασης βασίζεται κυρίως στη νευροπλαστικότητα του εγκεφάλου, μια εγγενή ιδιότητα η οποία εξασφαλίζει τη δυνατότητα δυναμικών τροποποιήσεων σε πολλαπλά επίπεδα νευρωνικής οργάνωσης. Αν και αυτή η νευρωνική ανάπτυξη είναι σε μεγάλο μέρος γενετικά προγραμματισμένη, η εμπειρία διαδραματίζει σημαντικό ρόλο μέσω της επιγένεσης, τροποποιώντας την έκφραση των γονιδίων με συνεπακόλουθες μεταβολές στην ανατομία και λειτουργία του εγκεφάλου και επιτρέποντας ακόμη και την τροποποίηση ορισμένων ελλειμματικών δομών.
