Περίληψη

Η έξω ωτίτιδα είναι η συχνότερη παθολογική κατάσταση του έξω ακουστικού πόρου στα μικρά ζώα με συχνότητα που είναι μεγαλύτερη στο σκύλο απ' ότι στη γάτα. Η πολυπαραγωντική της αιτιολογία περιλαμβάνει τους προδιαθεσικούς παράγοντες, τα προκαλούντα αίτια καθώς και τους παράγοντες εκείνους που ευθύνονται για τις συχνές υποτροπές. Στα σπουδαιότερα προκαλούντα αίτια περιλαμβάνονται η ατοπία, η τροφική αλλεργία, η ωτοδηκτική ψώρα, τα ξένα σώματα, οι ενδοκρινοπάθειες και οι διαταραχές της κερατινοποίησης. Τα βακτηρίδια και οι ζύμες, που σαπροφυτούν στον εξωακουστικό πόρο, συχνά επιπλέκουν τις αρχικές αλλοιώσεις, οδηγώντας έτσι στην εγκατάσταση κυψελιδοπαραγωγικής ή πυώδους έξω ωτίτιδας. Η μέση ωτίτιδα και οι υπερπλαστικές αλλοιώσεις του έξω ακουστικού πόρου είναι υπεύθυνες για τη διαιώνιση της νόσου. Η διάγνωση γίνεται εύκολα με την καλή διερεύνηση του ιστορικού, τον καθαρισμό του έξω ακουστικού πόρου και την ωτοσκόπηση, την κυτταρολογική και παρασιτολογική εξέταση των εξιδρωμάτων, την καλλιέργεια για βακτηρίδια και το αντιβιόγραμμα και την ακτινολογική εξέταση (τιτάνωση του χόνδρου, μέση ωτίτιδα). Η διαγνωστική διερεύνηση και η θεραπευτική αντιμετώπιση της υποκείμενης δερματοπάθειας κρίνονται απαραίτητες. Η θεραπεία της έξω ωτίτιδας εξαρτάται από την ταυτοποίηση και τον έλεγχο των προδιαθεσικών παραγόντων και των αιτιών, εφόσον βέβαια αυτό είναι δυνατό. Η συντηρητική θεραπεία συνίσταται στον καθαρισμό ή και στη συστηματική χορήγηση διαφόρων φαρμακευτικών ουσιών (ακαρεοκτόνα, αντιμυκητικά, αντιβιοτικά, γλυκοκορτικοειδή). Οι διορθωτικές χειρουργικές επεμβάσεις θα πρέπει να γίνονται στα περιστατικά εκείνα που δεν ανταποκρίνονται στη συντηρητική αγωγή.
